LPN/LVN Intake Coordinator - 20142255

I Am Texas Home Health

As a LVN Intake Coordinator you will:

Receive referrals and advise patients, family members, staff, and physicians about programs available for patient care needs.
Document referral orders with signature and date; ensures orders received by LVN/LPN are reviewed and cosigned by RN.
Comply with the applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA and Documentation Standards.

Requirements of Qualified Candidates

Graduate of accredited practical/vocational nursing program
Currently Licensed Practical/Vocational Nurse in state(s) of agency operation
Must possess and maintain valid CPR certification while employed in a clinical role
Two (2) years of experience in community health nursing setting with five plus (5+) years of progressively responsible nursing experience. Supervisory experience preferred
Previous Intake experience. Home Health experience preferred
Excellent verbal and written communication skills
Ability to demonstrate empathy, patience and compassionate nature
Must have the ability to function autonomously and possess effective time management skills
